"Treatment of Anemia with Darbepoetin Alfa in Systolic Heart Failure"
NEJM Mar 10
http://www.nejm.org/doi/full/10.1056/NEJMoa1214865
"CONCLUSIONS

Treatment with darbepoetin alfa did not improve clinical outcomes in patients with systolic heart failure and mild-to-moderate anemia. Our findings do not support the use of darbepoetin alfa in these patients."

"Coronary artery bypass graft surgery versus percutaneous coronary intervention in patients with three-vessel disease and left main coronary disease: 5-year follow-up of the randomised, clinical SYNTAX trial"
Lancet Feb 27
http://www.lancet.com/journals/lancet/article/PIIS0140-6736(13)60141-5/fulltext
"Interpretation

CABG should remain the standard of care for patients with complex lesions (high or intermediate SYNTAX scores). For patients with less complex disease (low SYNTAX scores) or left main coronary disease (low or intermediate SYNTAX scores), PCI is an acceptable alternative. All patients with complex multivessel coronary artery disease should be reviewed and discussed by both a cardiac surgeon and interventional cardiologist to reach consensus on optimum treatment."

"Use of high potency statins and rates of admission for acute kidney injury"
BMJ Mar 19
http://www.bmj.com/content/346/bmj.f880
"Conclusions Use of high potency statins is associated with an increased rate of diagnosis for acute kidney injury in hospital admissions compared with low potency statins. The effect seems to be strongest in the first 120 days after initiation of statin treatment."

NEJM Mar 21
http://www.nejm.org/doi/full/10.1056/NEJMoa1211716?query=featured_home
"CONCLUSIONS

Closure of a patent foramen ovale for secondary prevention of cryptogenic embolism did not result in a significant reduction in the risk of recurrent embolic events or death as compared with medical therapy. (Funded by St. Jude Medical; ClinicalTrials.gov number,NCT00166257.)"

Lancet Mar 16
http://www.lancet.com/journals/lancet/article/PIIS0140-6736(13)60354-2/fulltext
"Interpretation

Alcohol misuse and aggressive behaviour might be appropriate targets for interventions, but any action must be evidence based. Post-traumatic stress disorder, though less prevalent, is also a risk factor for violence, especially hyperarousal symptoms, so if diagnosed it should be appropriately treated and associated risk monitored."

NEJM Mar 11
http://www.nejm.org/doi/full/10.1056/NEJMoa1300610?query=featured_home
"CONCLUSIONS

Nonemergency PCI procedures performed at hospitals in Massachusetts without on-site surgical services were noninferior to procedures performed at hospitals with on-site surgical services with respect to the 30-day and 1-year rates of clinical events. (Funded by the participating hospitals without on-site cardiac surgery; MASS COM ClinicalTrials.gov number, NCT01116882.)"

"Randomized controlled trial to prevent postpartum depression in adolescent mothers"
AJOG Mar 2013
http://www.ajog.org/article/S0002-9378(13)00003-3/fulltext
"Results
Participants included Hispanic (53%), non-Hispanic black (17%), and non-Hispanic white (16%) adolescents. The overall rate of depression in the intervention group (12.5%) was lower than the control group (25%) with a hazard rate ratio of 0.44 (95% confidence interval, 0.17−1.15) at 6 months after delivery.

Conclusion

An intervention that is delivered during the prenatal period has the potential to reduce the risk for postpartum depression in primiparous adolescent mothers."

"Palliative use of non-invasive ventilation in end-of-life patients with solid tumours: a randomised feasibility trial"
Lancet Mar 2013
http://www.lancet.com/journals/lanonc/article/PIIS1470-2045(13)70009-3/fulltext
"Interpretation

Our findings suggest that NIV is more effective compared with oxygen in reducing dyspnoea and decreasing the doses of morphine needed in patients with end-stage cancer. Further studies are needed to confirm our findings and to assess the effectiveness of NIV on other outcomes such as survival. The use of NIV is, however, restricted to centres with NIV equipment, our findings are not generalisable to all cancer or palliative care units."
